Another Sugarbear season has been completed. Please take the time to complete an evaluation. We can only improve or change if we hear back from you. By completing and returning your evaluation, you become eligible for the random drawing for one FREE 2011 Team Registration Fee.

The Roseville Parks, Recreation & Libraries Department would like to thank you for taking the time to fill out this questionnaire in order to assist them with improving their programs in the future.

To be entered into a random drawing for (1) free 2011 team registration fee, please complete the evaluation and return it to the Roseville Sports Center, 1545 Pleasant Grove Blvd by August 31, 2010.
